What We Do

Disease Management Services

The Disease Management (DM) Program identifies Medicaid-eligible individuals with high medical costs who have a diagnosis of Serious Mental Illness (SMI) or substance use disorder (SUD) and are not currently receiving Division of Behavioral Health (DBH) behavioral health services.

 

Our outreach workers offer help in coordinating healthcare, reducing life stressors, and management of each persons medical, psychiatric, and substance use conditions. Coordination of care is at the center of our Disease Management program to ensure the best possible outcomes for this population.

 

We emphasize community support/case management services and help each participant by making our American Society of Addiction (ASAM) programs available.
